A Study of Antepartal Fetal Surveillance after 41 Weeks of Gestation




Abstract
Postterm pregnancy contributes as one of the important high risk factor in the obstertrical area and perinatal morbidity such as intrapartum distress, meconium aspiration, birth asphyxia, macrosomia is increased compared to normal term pregnancy.
Recent studies has shown that perinatal asphyxia in increased between 40 and 42 weeks of gestation and recommanded fetal well-being tests after 41 weeks of gestation. To evaluate and compare the efficacy of fetal well-being tests sach as NST,
CST,
AFV
measurement and umbilical artery Doppler velocimetry in predicting the poor perinal outcome after 41 weeks of gestation, 122 cases of noncomplicated pregnanct women were selected from period of June 1989 through June 1991 in our department.
The sensitivity of AFV measarement was 41.2%, NST 30.8%, CST 27.3%, and umblical artery velocimetry 5.6%, respectively. The specificity and positive predictve value were realtively high is all tests.
In our study, umblical artery Doppler has less value in prediton of fetal outcome in gestational age grcater 41 weeks AFV measurement showed better diagnostic diagnostic accuracy compared to conventional NST and CST and it can be usred as one of
the
important prognostic factors to predict the perinatal morbidity in possterm pregnancy.
